Adult REVIVE
(Ages 14 and up)
$400/year
- 2 professional dental cleanings per year
- 2 doctor exams per year
- 2 fluoride varnish treatments per year
- X-rays as needed with cleanings
- 15% off all other treatments, including Invisalign
Child REVIVE
(Up to age 13)
$350/year
- 2 professional dental cleanings per year
- 2 doctor exams per year
- 2 fluoride varnish treatments per year
- X-rays as needed with cleanings
- 15% off all other treatments
Family REVIVE
$1250/year
- Includes 2 Adults + 2 Children in same household
- $250 surcharge for each additional family member
- Same preventative benefits apply to each patient
- Same 15% off all other treatments
